MAKE AN APPOINTMENT TO SEE MY DOCTOR?
Patients are registered with the practice rather than with individual GPs.
However, when booking a routine appointment patients have the flexibility to request to be seen by the practitioner of their choice.
We would like to advise that patients may have to wait longer to see a preferred practitioner.
To make an appointment to see a doctor; telephone or call in to reception
between 8.30am and 5.30pm.
An appointment is for one person; if another member of the family wishes
to see the doctor please make two appointments.

OBTAIN A HOME VISIT?
If you are too ill to attend the surgery a home visit can be arranged by
ringing the surgery before 10.30am.

OBTAIN TEST RESULTS?
Please telephone between 2.00 and 4.00pm to enquire if results of
routine smear tests, blood tests, scans or x-rays are back. Patients must make an appointment to see the doctor who has requested the
tests to obtain the results.
PLEASE NOTE: test results will not be given over the telephone.

OBTAIN REPEAT PRESCRIPTIONS?
Repeat prescriptions may be issued to you if you require regular medication. Please allow at least two working days (48 hours) excluding weekends and bank holidays, for the prescription to be prepared.
PRESCRIPTION REQUESTS
- Tick the items required on the prescription counterfoil or repeat
prescription card and leave it in the box provided on the reception counter. Alternatively, you can post it to the surgery
but please enclose an SAE if you wish the prescription to be posted back to you.
- Ordering by telephone on the prescription line (01977) 465773 is between 11.00am and
1.00pm Monday to Friday - please state either your computer number or name and address
and the names of the items you require.

OBTAIN A SICKNESS CERTIFICATE?
If you are absent from work due to illness for less than seven days, you need to
obtain a Form SC2 from your employer or the DSS. If after seven days you are still unwell, you will need to see a doctor, who
may then issue a medical certificate. For illnesses of less than seven days, a private certificate can be issued if your
employer requires one. A fee is payable for this item. NB: duplicate sick notes will not be issued.
